Optimization of individualized therapy for colorectal cancers (CRCs) with secondary resistance towards targeted anti-EGFR treatment
- The clinical benefit of anti-EGFR targeted therapy is limited by the development of secondary resistance (SR) in virtually all treated CRC patients. Our global gene expression analyses identified a widespread transcriptional reprogramming of many genes that are potentially able to reactivate KRAS and PI3K signaling pathways. Here, fibroblast growth factors FGF13, FGF19, FGF20, and insulin-like growth factor IGF2 genes were chosen as promising candidates for mediating SR to Cmab monotherapy in CRC. We created transgenic CRC models using a lentivirus-based system to overexpress or downregulate the candidate genes. Our data showed that the overexpression of the GF ligands FGF19, FGF20, or IGF2 enhanced cell proliferation and conferred Cmab resistance in CRC cells. These data suggest that anti-EGFR therapy should be combined with either IGF1R or FGFR inhibitor therapy to control the growth of anti-EGFR SR CRC tumors activating IGF1R or FGFR pathway-related members under the EGFR blockade.
- Der klinische Nutzen einer Anti-EGFR-Therapie wird durch die Entwicklung einer sekundären Resistenz (SR) bei allen behandelten CRC-Patienten eingeschränkt. Unsere globalen Genexpressionsanalysen identifizierten eine transkriptionelle Reprogrammierung vieler Gene, die potenziel KRAS- und PI3K-Signalwege reaktivieren. Dabei wurden die Wachstumsfaktoren FGF13, FGF19, FGF20 und IGF2 Gene als Camb SR Kandidaten ausgewählt. Wir erstellten transgene CRC-Modelle unter Verwendung eines Lentivirus- basierten Systems zur Überexpression der Kandidatengene. Unsere Daten zeigten, dass die Überexpression der GF-Liganden FGF19, FGF20 oder IGF2 die Zellproliferation und Cmab-Resistenz in CRC-Zellen verstärkte. Diese Daten legen nahe, dass eine Anti-EGFR-Therapie entweder mit einer IGF1R- oder FGFRR-Inhibitortherapie kombiniert werden sollte, um das Wachstum von Anti-EGFR-SR-CRC-Tumoren zu kontrollieren, die IGF1R- oder FGFR-Signalweg-bezogene Mitglieder unter der EGFR-Blockade aktivieren.
