• \(\bf Studienziel:\) Die Verwerfrate von Eigenblutspenden vor Hüft- oder Knie-TEP`s ist hoch. Es wurden der Hämoglobin ( Hb )-Wert und das Alter der Patienten vor Durchführung einer Eigenblutspende und deren Einfluss auf den Blutbedarf untersucht. \(\bf Methode:\) Retrospektiv wurden die Daten von 233 Patienten analysiert, denen eine Hüft- oder Knie-TEP implantiert worden war und die präoperativ Eigenblut gespendet hatten. \(\bf Ergebnisse:\) 72 Patienten bekamen ihr Eigenblut retransfundiert. Im multivariaten Modell konnte kein Einfluss des Alters auf die Notwendigkeit einer Eigenbluttransfusion nachgewiesen werden, aber mit einem höheren Ausgangs-Hb-Wert sank das Risiko signifikant pro Einheit auf das 0,712fache. Somit war weniger das Alter der Patienten sondern mehr der Ausgangs-Hb-Wert aussagekräftig ob eine Transfusion notwendig wurde oder nicht. \(\bf Schlussfolgerung:\) Die Entscheidung zur Eigenblutspende muß individuell getroffen werden. Hierbei ist der Ausgangs Hb-wert der zuverlässigste Parameter.
• \(\bf Aim:\) Rejection rate of autologous blood donation before joint replacement is high. Influence of the haemoglobin value and age of patient before autologous blood donation was examined according to the necessity for blood transfusion. \(\bf Method:\) In a retrospective study, data of 233 patients who had donated autologous blood before hip or knee arthroplasty were analysed. \(\bf Results:\) 72 patients received an autologous blood transfusion during surgery or in the further course until the first day after surgery. A multivariate analysis showed no significant influence of age on the need for transfusion, but a higher haemoglobin value before blood donation reduced the risk significantly to 0,712 per unit. Therefore the age of the patients was less predictive compared to the haemoglobin value as to whether or not a blood transfusion had been necessary. \(\bf Conclusion:\) The decision for autologous blood donation has to be made on diverse criteria. Therefore is the haemoglobin value the trusties parameter.
