Predicting and modulating pain
- While advances in brain imaging have provided valuable knowledge on how the brain responds to pain, there is a lack of means to predict pain, as well as a lack of non-invasive approaches to modulate it. Therefore, the three main objectives of this thesis are (1) to identify objective predictors of pain sensitivity for healthy subjects, (2) to identify clinical pain predictors of ictal migraine pain, and (3) to evaluate the potential of transcutaneous spinal direct current stimulation (tsDCS), a non-invasive technique, to suppress pain. To this end, a total of 145 participants were recruited across three different studies. This thesis provides novel evidence that a large portion of both healthy and migraine-related pain sensitivity can be predicted by spontaneous brain fluctuations in resting state networks. Furthermore, anodal tsDCS was shown to suppress mechanical pain sensitivity and alter functional connectivity strength.
- Trotz wertvoller Fortschritte in der Hirnbildgebung bzgl. des Wissens wie das Gehirn auf Schmerz reagiert, brauchen wir noch prädiktive Indikatoren für Schmerz, sowie non-invasive Verfahren, die Schmerz modulieren. Meine Dissertation behandelt diesbezüglich drei Zielsetzungen: (1) Die Identifikation objektiver Schmerzprädiktoren in gesunden Kontrollprobanden, (2) Die Identifikation objektiver klinischer Schmerzprädiktoren bei Migränepatienten, und (3) Die Evaluation transkutaner spinaler Gleichstromstimulation (tsDCS) als non-invasives Verfahren der Schmerzlinderung. Insgesamt wurden 145 Probanden für 3 verschiedene Studien rekrutiert. Meine Ergebnisse bieten Anzeichen dafür, dass ein signifikanter Anteil des Schmerzes in Kontrollprobanden und Migränepatienten durch spontane Hirnaktivität in Hirnnetzwerken vorausgesagt werden kann. Abschließend konnte ich belegen, dass anodale tsDCS einen Linderungseffekt auf mechanische Schmerzsensitivität hat.
